African on moon, after complex double hip replacementThe latest minimally invasive hip replacement procedure – Direct Hip Anterior (DHA), performed for the first time in IndiaHyderabad, March 22nd, 2018: Orthopedic Surgeons at Apollo Health City, Jubilee Hills, led by Dr P Sharat Kumar, Consultant Orthopedic Surgeon, provided succor to a 53-year-old Somalian patient, Mr Mukhtar Hilowli, by performing a double hip replacement using the latest minimally invasive procedure – the Direct Hip Anterior, for the first time in India. This was announced at a press conference by Dr P Sharat Kumar and Dr Hari Prasad, President, Hospitals Division, Apollo Hospitals.  Both the hips of Mr Mukhtar Hilowli got fused following a severe bout of infection, way back in 1986. The hip joint comprises of femoral head (ball like) and acetabulum (socket). For easy movement of the hips the femoral head moves in the acetabulum. In his case due to infection in both right and left hip joints the normal cartilage on femoral head and acetabulum was destroyed and caused fusion of femoral head and acetabulum.The two bones at the hip joint fused and became one, depriving him of the normal hip joint movement, says Dr Hari Prasad.The fusing of hips joints led to his hips losing the natural maneuverability and made his day to day life miserable. He had an abnormal gait on account of this, which made movement on his two legs arduous. His normal routine was impaired during the prime of his life and had to endured the trauma for the last 31 years. His ordeal only amplified in recent times, as he also developed pain in both knee joints and lower back, which was due to his abnormal posture and excess stress on these joints.After exhausting various options of treatment, he approached Dr Paripati Sharat Kumar on December 29th, 2017. After thorough examination and investigations, Mr Hilowli underwent sequential bilateral total hip replacements, with the right hip being replaced first on December 30th, 2017, followed by the left hip replacement on February 15th, 2018.According to Dr Sharat, the conversion of a fused hip joint is extremely complex and challenging, due to excess blood loss, poor bone and soft tissue quality and difficulty in orientation as evidence of joint surface is completely obliterated. As the joints had fused more than 3 decades ago, the surgery itself was quite demanding.In a first of its kind in India, Dr Sharat used the latest and minimally invasive procedure –Direct anterior approach. This is considered the most preferred choice in hip surgery off-late, globally. The conventional approach involves making a long incision and cutting muscle that support and stabilize hip joint. While in direct anterior approach, the muscles areseparated to reach and expose the hip joint through a 3 to 4-inch incision on the front of the hip. He elected this approach as the patient’s strength was poor around his hips and the joint stability would be further compromised by the conventional approach. Also pain and bleeding is relatively less and rehabilitation is quicker. The patient has since recovered well and is undergoing physiotherapy and rehabilitation. The patients were also present on the occasion.Mr Biswajit Saha, a 37 year old farmer hailing from Assam, was suffering from rare bone marrow stem cell disorder called PNH (Paroxysmal Nocturnal Haemoglobinuria), which occurs in 1 amongst a 10 lakh population. He was suffering with Anaemia, red colored urine due to breakage of his red blood cells for the last two years. Such patients develop blood clots in the main blood vessels and if left untreated can develop life threatening heart and lung problems. Saha was transfused with blood at regular intervals, at times 3 units a week. His visits to several doctors couldn’t get him relief and was mostly misdiagnosed and treated for a liver ailment. His condition deteriorated over time as he had developed lung and heart problems and was breathless on minimal exertion. That’s when he approached Apollo Cancer Institute, Hyderabad and our diagnosis identified it as PNH. The option available for Saha was an antibody treatment called Eculizumab(solaris), to be taken every week for lifetime. This medication is not available currently in India and is also way beyond his means, costing Rs 8 lakhs per week. Therefore the second-best treatment option was bone marrow Transplant. He has undergone the first PNH transplant in India (to the best of our Knowledge) four months ago, with the bone marrow donated by his brother. He has since recovered fully and is going about with his daily routine, in his village. Since the transplant he has not needed any blood transfusion and his lung and heart functions reverted to normal.A thirty one year old mother of two, Mrs Kavitha, was diagnosed with Biphenotypic Acute Leukemia, a lethal ailment with two different types of blood cancers – acute Myeloid and Acute Lymphoid Leukemia, manifesting simultaneously. She had 98% disease in the bone marrow, in the chest and abdomen, cancer cells were detected even in the brain. The survival rate of patients with this type of leukemia was a dismal 8%. The initial dose of chemotherapy proved futile as the disease in her was virulent and had to take recourse to administering two other aggressive types of chemotherapy to gain control over it. However, the permanent cure was to perform a Bone marrow transplant on her. The strong dosage of chemotherapy took a serious toll on her condition and had to be infused with nutritional support through her veins daily, to improve her weight and prepare her for a Transplant. The next seemingly insurmountable challenge we had on our hand was not finding a matching donor from amongst her family members and our attempt to locate a full match for her in the Indian, German and American registries proved in vain. We had to compromise with a donor of a different blood group with 90% match from Bangalore. The Transplant performed with all the associated challenges was of very high risk and extremely complex. Subsequent to the transplant she contracted Cytomegalovirus virus infection, she fought through it and has now crossed 100 days which is a giant milestone in the transplant process. Her blood test reveals 99% of Donor DNA type which gives reassurance that her Leukemia is cured.The third patient, Mrs Prathima a 32 year old lady was diagnosed with Aplastic anaemia three years ago and had very low platelet count and anaemia. Such patients are highly susceptible to infections and need regular blood and platelet transfusion. The initial option of Horse serum (ATG) treatment did not have long lasting success. The only chance of cure for her was bone marrow Transplantation, for which she came to Apollo Cancer Institute, Hyderabad. She didn’t have a matching family donor, our search for a full 100% matching donor helped locate one in Germany. We had donor stem cells collected and airlifted fresh within 24 hours from there. Since her transplant she had not had any major complications and didn’t need any platelet transfusions.It’s a moment of immense fulfillment for me and the entire team at Apollo Cancer Institute, Hyderabad, for successfully treating patients with three complex and rare blood cancer ailments needing bone marrow transplantation. 29 year old man undergoes successful surgery for rare tracheal (windpipe) tumor at Indraprastha Apollo HospitalsPatient presented with breathlessness, chest discomfort with SPO2 of 70%The tumor had blocked the wind pipe causing a life-threatening situation19th December, 2022, New Delhi: In a rare case, a 29 year old man was successfully operated for a tumor in his windpipe, at Indraprastha Apollo Hospitals, New Delhi. Initially, the patient had complained of chest pain and progressive swelling in his neck. After suffering for over 2 months he consulted a local doctor who diagnosed him with Tuberculosis, but his condition did not improve with TB medication. As next steps he underwent bronchoscopy that revealed a large mass in his windpipe causing 80% blockage. With his condition deteriorating due to acute breathlessness and fluctuating oxygen levels he was admitted to a local hospital of Faridabad.With no respite in his condition he was referred to Indraprastha Apollo Hospitals, New Delhi where he was admitted under the supervision of Dr Nikhil Modi, Senior Consultant, Respiratory and Critical Care Medicine, Indraprastha Apollo Hospitals, New Delhi. Here he was immediately put on ventilator to first stabilise his oxygen level, however when further examined by Dr Suresh Singh Naruka, Senior Consultant, ENT and Head and Neck Surgeon, Indraprastha Apollo Hospitals, New Delhi for his tumour, he was recommended an immediate tumor removal surgery.Speaking on the same, Dr Nikhil Modi, Senior Consultant, Respiratory and Critical Care Medicine, Indraprastha Apollo Hospitals, New Delhi said, “Early and correct diagnosis has the best opportunity for a positive heath outcome. Sometimes, with patient getting delayed medical care the condition can become life-threatening. Same was the case with this 29 year old man from Uttar Pradesh, due to delay in treatment & wrong diagnosis, the tumor increased in size and caused nearly complete blockage of the wind pipe. With his condition deteriorating he was admitted on 29th October, 2022 at Indraprastha Apollo Hospitals. To maintain his airway he underwent urgent tracheostomy and bronchoscopy followed by tumor removal surgery.”Dr Suresh Singh Naruka, Senior Consultant, ENT and Head and Neck Surgeon, Indraprastha Apollo Hospitals, New Delhi said, “When this patient was admitted to Apollo he had developed a tumour which was causing 80% blockage of the neck. Hence after stabilising his oxygen levels an immediate surgery was carried out in order to maintain the airway. This surgery in particular was challenging. Due to the size of the tumour, there was no space for air to pass into the lungs and patient saturation rate was dropping very fast whenever we were doing any instrumentation. The procedure took 3 hours but were able to successfully remove the tumour. Postoperatively the patient was kept in ICU for further monitoring and as he was successfully weaned off from mechanical ventilation, he was discharged on 9th November, 2022. Post- surgery, the biopsy of wind pipe mass revealed a cancer of lymphoid cells known as Lymphoma. Lymphoma is extremely rare to find in wind pipe as it comprises only 0.2% to 0.3% of all wind pipe tumors. Basis the presence of lymphoma he is undergoing chemotherapy as further management and is keeping well now.”The patient is still visiting the hospital for follow-ups and management of Lymphoma through chemotherapy.

Hyderabad, May 16th, 2017: World Hypertension Day (WHD) is observed on 17th of May every year to raise awareness on Hypertension and World Hypertension League (WHL) ensures its observation world over. The theme of WHD is ‘Know Your Numbers’ with an aim to increase awareness on high blood pressure. The theme being pertinent considering that only 50% of those with hypertension are aware of their condition. In certain communities the awareness is as dismally low as 10%. Of those who are diagnosed, less than half are treated and only a small percentage achieve normal BP levels. WHD is to raise the awareness levels and make individuals conscious about hypertension and the necessary precautions. To live long and free of disease, one needs to keep the BP normal always—24 hours a day. No exceptions, he says.National studies have shown that nearly 30-35% of adults in Hyderabad and Telangana have high BP levels. But sadly, only less than 10% know that they have high BP. Unless we control high BP in Hyderabad and in our state, danger is looming, a Tsunami is heading our way. All this suffering can be avoided through awareness, early diagnosis, and correct management of high BP. People should be aware that unless checked, they may be in the dark. GET THE BP CHECKED. And be treated. Costs including financial/social/societal of untreated high BP are astronomical. According to estimates the annual cost of medical care of patients with hypertension related disease in India is $20 billion. Vascular surgery, heart surgery, dialysis, transplantation, and stroke rehabilitation are very expensive. So, be smart and prevent these complications by keeping your BP normal. With life-style changes, dietary caution, and medicines, it is possible to lower your BP. But if the BP is neglected or not treated early on, it is tough to get it under control. So, the key is awareness, early diagnosis, and early treatment. Waiting is harmful. BP will not evaporate, it stays and gets worse.Mind you, BP has to be treated indefinitely. If you are on the medicine, you have to take it indefinitely and not cyclically. BP medicines are not like antibiotics, you should not stop the medicines unless advised by the doctors. Patients assume that the treatment is only for few days and then, they stop. No, BP treatment is not seasonal or as you wish. You must remain on treatment unless there is a reason and that too only under medical supervision. Don’t take life in your hands; don’t go through the red signal of high BP. Go through when the signal becomes green, which is normal BP.In some patients, the high BP could be because of “secondary” hypertension, like Thyroid gland disease, adrenal gland problems, blockage of circulation to the kidney, certain kidney ailments. Heavy snoring/ sleep apnea can cause very high BP and the remedy is to treat the snoring disorder. BP problems in pregnancy can be extremely dangerous for the mother and the fetus. Excessive alcohol use or pain relievers can cause high BP. Patients with secondary hypertension can be cured or improved by correcting the underlying cause. The doctors can suspect a secondary cause and if it is fixed, BP will become normal.It also manifests differently in different people, in some the blood pressure goes up only in the doctor’s office or hospital, but is normal outside (at home or work), this is called “white coat hypertension.” Nervous individuals, and “tense” people may have white coat hypertension. In some others, the blood pressure is normal in the doctor’s clinic or hospital but is high outside (at home or work). This is called “reverse” white coat hypertension or “masked” hypertension. We can diagnose these abnormal condition by measuring 24 hr blood pressure (at home or work) with an automatic machine.Unfortunately, high BP is striking young individuals in our city, the condition is turning alarming and a cause for great concern. If we don’t take care of this immediately, the young are at a risk of stroke/paralysis, heart attack, and kidney failure. After a detailed evaluation he was diagnosed to have Gastroesophageal reflux disease (GERD) with a hiatus hernia. He underwent surgery in the form of fundoplication (wrapping of part of the stomach around the lower esophagus) in his home country. However soon after surgery he developed severe difficulty in swallowing both solids and liquids (dysphagia). Two more surgeries were done within a span of 6 months with the aim of relieving the symptoms but unfortunately after each surgery the symptoms only worsened. His discomfort progressed to a stage where after every meal he would have a feeling of the ingested food being stuck in the chest and upper abdomen. This would progress to severe discomfort and be relieved after vomiting. This symptom has resulted in a fear of eating and significant weight loss.At this stage he came to IAH and a detailed evaluation was done which included endoscopy and CT scans of the chest and abdomen. These revealed that subsequent to the prior surgical procedures most of his stomach was lying in the chest cavity instead of its normal position in the abdominal cavity. This was also resulting in a marked angulation of the stomach because of which ingested food was not freely passing into the intestine causing his symptoms of severe discomfort and vomiting after ingestion of food. Since he had undergone multiple previous surgeries, he was advised open surgery instead of minimally invasive laparoscopic surgery. At the time of surgery both his chest and abdominal cavities were required to be opened to allow proper access. The stomach was straightened and pulled down to its normal position to allow ingested food to pass down to the intestine normally.His post-operative recovery after the complex surgery was uneventful and he has since then returned to his country.Total Femoral Prosthesis, An Extremely Uncommon Surgery Performed By Dr Raju Vaishya.Successful salvage of a severely crushed leg and ankleA 30 year old male from Oman, presented following a Road traffic accident on 8/07/2022. He had sustained a severe, crush injury of left leg and foot, for which was initially managed in his country, but as his limb condition worsened, he was referred to Indraprastha Apollo Hospitals, New Delhi. He was advised amputation in view of h poor limb condition and significant bone loss of about 3 inches. The extensive soft tissue damage was managed by the plastic surgery team of Dr Rajesh Watts. The limb salvage procedures were done, in the form of wound debridement, external fixation of the fractures, free flap cover of the wound, limb lengthening, and finally the hindfoot arthrodesis was done on 31/01/2023, by Dr. (Prof.) Raju Vaishya’s team.All the wounds of his leg and foot have now healed, the leg and foot deformities are corrected. The leg has also been lengthened by 3 inches using Ilizarov’s fixator. While the donor was discharged within a week, the recipient was d r discharged after 20 days of the transplant.A resident of Bihar, 45-year-old male, underwent a successful liver transplant at Indraprastha Apollo Hospitals, New Delhi. He had been suffering from chronic liver diseases for the past two years. His condition deteriorated in the month of June this year, after he was diagnosed with jaundice, followed by persistent high fever and internal bleeding in the abdomen.Following his deteriorating health, he was admitted at Indraprastha Apollo Hospitals in emergency. In India the burden of end organ disease is steadily on the rise. Transplantation of Liver, kidney, heart, Lungs, pancreas and intestines has become the standard of care for the treatment of the respective organ failures. The surgical procedure and post-surgery care has been standardized to a great extent and organ transplantation has success rates of 90-95% across all organ types. However, inspite of the good outcomes, transplantation is done in less than 5% of all patients who need it, the biggest hurdle being availability of suitable organs for transplantation.  Organs for transplantation have to come from humans and presently, the majority of transplantation in our country are being done using live donor organs (from people who are alive). One of the kidneys and a part of the liver can be donated by a healthy person. Live donation, in addition to putting the donor at risk of complications (however small the risk may be), also makes transplantation of heart, lung and pancreas impossible as these organs cannot be donated by living persons.
