Apollo Hospitals Chennai Achieves Breakthrough with BASILICA-Assisted TAVI, Safeguarding High-Risk Heart Patient

Apollo Hospitals, Chennai, has successfully treated a 67-year-old high-risk heart patient using an advanced, minimally invasive procedure called BASILICA-assisted TAVI. This specialised technique is designed to prevent blockage of the coronary artery during valve replacement. This milestone highlights the growing role of advanced transcatheter therapies in managing complex structural heart disease, particularly in patients who are not suitable candidates for repeat open-heart surgery.  The procedure was performed by a multidisciplinary team led by Dr. Sengottuvelu G, Senior Interventional Cardiologist and Clinical Lead of Structural Heart Interventions at Apollo Hospitals Chennai. This approach offers a minimally invasive alternative for redo valve replacement cases, where conventional surgery carries significant risk. The patient had a history of diabetes, coronary artery disease with prior stenting, and aortic valve replacement surgery in 2017. Degeneration of the previous valve led to recurrent aortic stenosis (narrowing) and regurgitation (leakage), both of which can affect blood flow. Before the procedure, a scan revealed a high risk of blockage (an uncommon but serious complication of TAVI), associated with mortality rates of up to 40–50% when it occurs. To reduce this risk, doctors used a combination of TAVI (a catheter-based valve replacement procedure) and the BASILICA technique. In simple terms, BASILICA involves carefully splitting a part of the old valve using controlled energy so that it does not block blood flow to the heart during the new valve placement.   Dr. Sengottuvelu G noted, “This case highlights the evolving capabilities of transcatheter therapies in managing highly complex structural heart disease. BASILICA is a technically demanding procedure, and its successful execution reflects the expertise and coordination of our team.” Dr. C S Muthukumaran, Interventional Cardiologist at Apollo Hospitals Chennai, added, “Performing the first BASILICA procedure in the Apollo Group is a proud moment. It reinforces our commitment to bringing cutting-edge, life-saving technologies to patients in India.” After the procedure, the patient showed good recovery, with improved blood flow, stable rhythm, and no evidence of heart blockage, and was discharged in stable condition. With international success rates exceeding 90% in selected cases, BASILICA remains a highly specialised procedure, underscoring Apollo Hospitals’ commitment to delivering evidence-based, patient-centric cardiac care.

Team from Apollo Chennai Uses Robotic Technology to Treat Severe Spine Deformity in South American Teen

When a 15-year-old girl from Suriname, South America, developed adolescent idiopathic scoliosis, an abnormal curvature of the spine with no clearly identifiable cause, her family began an urgent global search for specialised care. With limited treatment options in their home country, they reached out to multiple international centres. Apollo Speciality Hospitals, Teynampet, Chennai, responded promptly, enabling a swift consultation and guiding the family’s decision to travel to India for treatment. Dr. Sajan K. Hegde, Director, Spine Surgery, Apollo Speciality Hospitals, and the team, with the 15-year-old patient- Sedi Van Genderen, and her mother - Iris. On arrival, the patient was diagnosed with a severe and rapidly progressing spinal deformity that was affecting posture, mobility, and long-term health. Under the leadership of Dr. Sajan K. Hegde, Director – Spine Surgery, the multidisciplinary team adopted a staged, technology-driven approach. The initial phase involved halo gravity traction over three weeks to gradually improve spinal alignment. This was followed by a two-stage surgical intervention supported by robotic navigation and intraoperative neuromonitoring. In the first surgery, screws were placed into the spine using robotic guidance to ensure high precision. In the second surgery, damaged discs were removed and the spine was stabilised by fusing certain bones together. Finally, the spine was carefully straightened and fixed in place across multiple levels. Robotic systems helped doctors see the spine in real time and place implants with great accuracy, improving outcomes and reducing risks. Following the surgeries, the patient’s spinal curvature was corrected by about 75%, said Dr. Appaji Krishnan, Senior Consultant – Spine Surgery, who emphasised that the outcome was the result of a coordinated multidisciplinary team effort. The patient is recovering well, underscoring the impact of precision-led technologies and expert collaboration in managing complex spinal deformities, and reinforcing Chennai’s position as a trusted global destination for advanced spine care.

Apollo Cardiologists provide succour to a 61 year old with fatal heart condition using advanced techniques of ...
Hyderabad, September 23rd, 2021 : A team of Cardiologists led by Dr Karunakar Rapolu, Consultant Cardiologist, Apollo Hospitals; performed a complex interventional procedure using Impella, IVL and OCT; on an extremely critical 61 year old patient. The patient had 90% blockage due to high calcium deposit in the Left Main Vessel, which was jeopardising the blood supply to already severely weakened heart. A preliminary medical evaluation found him unsuitable for a bypass surgery, due to his past history of Coronary Artery Disease and having undergone an angioplasty three years ago. The Team had to swiftly explore for options to restore blood circulation before his condition deteriorated further and became fatal. The patient Mr Ravinder Reddy, a retired police official, presented with excruciating chest pain and symptoms of severely restricted blood flow to the heart due to calcification of the largest coronary artery, the Left Anterior Descending Artery (LAD). The LAD became narrow progressively since his previous angioplasty and manifested in the form of a massive heart attack. At the Hospital the patient was quickly wheeled into the Cath lab for an angiogram to assess the extent of heart damage. However, he developed dysfunction of the Left ventricular heart chamber, on the table, leading to sparse supply of blood to the vital organs and potential damage of those if not acted instantaneously. This episode further complicated his condition and the team had to resort to administering diuretics to rid excess fluids from the patient’s body and vasodilators to open the blocked vessels. His perilous condition ruled out any scope of performing the risk laden bypass surgery. The team narrowed down on the most apt option considering his delicate condition, of performing a CHIP Intervention (Complex High-risk Indicated Procedure) along with an Impella Heart Pump. CHIP Intervention is a highly critical procedure performed for immediate restoring of blood supply in the circulation system, while Impella replicates the work done by Left Ventricle, helps suck the oxygenated blood from the Left Ventricle and pumps it to the circulatory system. Using the infrared light guidance from OCT (Optical Coherence Tomography), the extent of occlusion was assessed and those high resolution images facilitated clearing of the blockage with precision. Intravascular lithotripsy (IVL) Balloon was used to crack the impregnable calcification of the blockage. IVL deploys acoustic pressure waves to break the calcium deposits. A stenting was done of the Left Main Coronary Artery using DK crush technique for the best outcome.During the course of the procedure the patient suffered 7-8 episodes of “Ventriculo arterial uncoupling” while inflating the balloon to clear occlusion, where the left ventricle ceased pumping the blood. Impella got into action and ensured uninterrupted supply of blood. The patient would have crashed during the procedure if Impella was not availed. The procedure took four and half hours, patient was moved out of ICU after 24 hours of the procedure. He has since recovered and was discharged on the 4th day of admission. He needs to take regular medicines and to follow healthy life style habits like regular exercises and diet management. Patient tolerated the procedure very well because of Impella heart pump which was working continuously to protect him, Oct guided the team to be precise, while IVL helped in the success of the procedure, says Dr Karunakar Rapolu.About Apollo Hospitals:It was in 1983, that Dr. Prathap C Reddy made a pioneering endeavour by launching India’s first corporate hospital – Apollo Hospitals in Chennai. Now, as Asia’s foremost trusted integrated healthcare group, its presence includes over 12,000 beds across 72 Hospitals and 4100 pharmacies, over 120 Primary Care clinics and 650 Diagnostic centres, 700 plus Teleclinics, over 15 medical education centres and a Research Foundation with a focus on global Clinical Trials. Apollo Hospitals performs a complex double organ – Heart & Lung Transplantation!
Hyderabad, July 2021: A team of doctors led by renowned Cardiothoracic Surgeon and Padmasri awardee Dr. Alla Gopala Krishna Gokhale performed a rare, combined Heart and Lung transplant on 20th July 2021 to save a terminally ill twenty-five-year-old woman, Ms Labdhi Jain. The recipient was unfortunately born with congenital heart disease with a hole in her heart known as a ventricular septal defect and an abnormal connection between her great arteries (Aorto-pulmonary window). Over the years, the congenital defect led to further deterioration in her health condition with increased lung pressures and leakage of heart valves. She was surviving on oxygen support at home for the last two-and-half years. Of late, she was admitted at various hospitals several times for heart failure. The transplant team at Apollo Hospitals, Hyderabad, on evaluation found her to be in an advanced stage of heart failure coupled with increased lung pressures. The team prescribed combined Heart and Lung transplantation to save her life. She was registered under the State Government’s Jeevandan Cadaver Transplantation program and was waiting for a suitable donor. The team faced unique challenges in this case. The recipient at the time of listing for transplant weighed only 30 kgs. Getting a suitable organ which was of appropriate in size for her was complex, as her heart cavity was small. She was helplessly waiting for a suitable organ for a prolonged period of over two and half years, with the hope of finding a small-sized organ diminishing by the day.The cadaver heart and lung obtained from an eighteen-year-old girl, pronounced brain dead due to intracranial bleeding and thrombosis, aptly suited her requirement. Despite the immense sorrow of losing their child, the donor’s family displayed exemplary courage and benevolence and gave consent to harvest their braindead daughter’s organs. This humanitarian gesture on behalf of the grief-stricken loved ones brought joy back in the life of another family. The recipient recovered well after surgery; her weight has improved to 38 kgs. She is being discharged from the hospital, freed from the burden of heart and lung disease, to enjoy life the way she deserves, and to overcome any obstacles that come her way with confident strides and a happy heart.About Apollo HospitalsIt was in 1983, that Dr. Prathap C Reddy made a pioneering endeavor by launching India’s first corporate hospital – Apollo Hospitals in Chennai. Seven year boy ‘Wilson’s Disease’ Transplanted with Liver
A seven year old boy with rare ‘Wilson’s Disease’ and fighting for life, was transplanted with liver at Apollo Hospitals!Hyderabad, February, 2017: Transplant Team at Apollo Hospitals performed liver transplant on a kid fighting for his life due to a genetic disorder – Wilson’s Disease and acute Liver failure, needing immediate medical intervention. The surgery was performed against all odds, using a slice from his mother’s liver.  Seven year old, Master T. Siddhartha Reddy, from Jeedimetla, was fighting for his life when he was first seen by Pediatric Hepatologist at Apollo Hospitals, Jubilee Hills, Dr Ramesh Srinivasan. Investigations revealed that he was suffering from a rare ailment called Wilson’s disease and Acute Liver Failure. Wilson’s disease is a genetic disorder in which copper builds up abnormally in the liver, eyes and brain. This overload of copper in the liver results in Liver failure. Often an otherwise apparently normal child can suffer with life threatening acute liver failure in this setting, warranting urgent liver transplantation, says Dr Ramesh Srinivasan. Majority of the people with the Wilson’s gene will be carriers of the gene and will not manifest the disease. It all began in the last week of December when Siddhartha Reddy was found to have jaundice. A spell on fever on the New Year day made his condition worse and the already diseased liver gave up entirely. His blood clotting parameters became abnormal and the INR – a measure of the blood’s ability to form clots; climbed up to 6.7, the normal value should be 1. When the liver is diseased this value increases and such patients are at a very high risk of bleeding at multiple sites in the body, including the brain. The alarming parameters prompted Dr Srinivasan to involve the liver transplant team at Apollo Hospitals, Jubilee Hills, Hyderabad.“When we saw the patient, he had very high bilirubin levels and his clotting parameters were so abnormal, we had to give him Fresh Frozen Plasma to keep his INR in the safe limit”, says Dr Manish Varma, Chief Liver Transplant Surgeon at Apollo Hospitals, Jubilee Hills.Siddhartha was immediately listed in the super-urgent category with Jeevandan. However, the team of doctors knew that they could not wait indefinitely and the patient was fast running out of time. Thankfully, the boy’s mother, a homemaker, had a matching blood group and the liver was found to be fit for donation. The next challenge to overcome was thelong weekend to follow starting with Sankranti holiday, necessitating completion of all the tests and the medico-legal paperwork within a day. The entire team including the Social work department worked overtime to make this happen and the medical tests for the donor, the legal paperwork were done within the day and the hospital authorization committee was requested to give an expedited clearance for the operation. The liver transplant took place in the early hours on 14th January, 2017 and was performed by a team led by Dr Manish C. Varma. However, for the transplant team challenges were far from over. The part of the liver that the mother donated, came with two very small blood vessels. The size of the vessels was 1 mm and 1.5 mm. “It is technically very challenging to join blood vessels that are small and the chances of they clotting again in the post-operative period was also very high, explained Dr. Manish Varma. Further the boy had to be given blood thinning medicines immediately after the transplant, which increases the risk of post-operative bleeding. How the highly experience Transplant team at Apollo Hospitals, weathering all the challenges and triumphantly completed the surgery. The boy has since recovered and has been discharged.

    Apollo Cancer Conclave
‘Apollo Cancer Conclave’ throws light on new innovations that will immensely benefit the patients & aid physicians to effectively treat cancersHyderabad, February 8th, 2017 : The 3rd edition of APOLLO CANCER CONCLAVE & 7th edition of their biennial CANCER CI – 2017 conducted from 2nd to 5th February 2017, at HICC, Hyderabad, was a resounding success, with a record 2340 delegates attending it. Globally acclaimed faculty comprising of Basic research Scientists, site specific Cancer experts, presented their data, participated in patient specific case discussions, issue based discussions and highlighted several recent innovations.  The advent of these new innovations will immensely benefit the patients and aid treating physicians to effectively combat cancer.Robotic SurgeryNewer techniques of Robotic Surgery which can be used for newer areas and with more ease, were presented.TOMOTHERAPY & Proton therapyAdvanced Radiation techniques, Tomotherapy & Proton therapy will drastically bring down the treatment duration and will replace Surgery in several areas of Cancer treatment. Also the whole body Radiation, whole marrow Radiation, multiple site treatment can be done in one go, making treatment less traumatic.Liquid BiopsyThe hallmark of modern era cancer care is ‘Tailored therapy’ as it has been identified that ‘every patient is different.’ This tailored therapy relies on the determination of molecular targets on the tumour.Liquid biopsies are non-invasive blood tests that detect circulating tumour cells and cell free tumor DNA, that are shed from the tumour and thus compliment an invasive biopsy.Liquid biopsy can be used to identify these targets and modify treatments, apart from their primary role in diagnosis. It also helps in identifying sensitivity of tumour to various chemotherapeutic agents. Liquid biopsy can also be used to predict the toxicity of a particular therapeutic agent by pharmacogenetic analysis.Pharmacogenomic ProfilingMost cancer drugs are effective in only a fraction of patients who take them. Identification of this responsive subset of patients would spare useless cytotoxic treatment. This sensitivity is identified by pharmacogenomics profiling. This can be done on tumor tissue or on Liquid biopsy.Genetic Tests in disease & screeningGene mutations that lead to cancer may be inherited from a parent or may happen spontaneously during a person’s lifetime. Genetic testing is the process of using medical tests to look for these changes in a person’s genes lost chromosomes. Genetic testing is used to see if a person has certain gene mutates known to increase the rest for a certain cancer eg. BRCA mutation testing in Breast Cancer.Immunotherapy, also called biologic therapy is a type of Cancer treatment designed to boost the body’s natural defenses to fight cancer. It uses substances either made by the body or in a laboratory to improve or restore immune system function.Checkpoint Inhibitors (CPI) block normal proteins on cancer cells, or the proteins on the T-Cells that respond to them. CPI seek to overcome one of Cancer main defenses against an immune system attack.Cancers which can be treated with Immunotherapy areMelanoma – Nivolumab and Pembrolizumab.Hodgkin’s Lymphoma – NivolumabHead & Neck Cancers – NivolumabLung Cancers – Nivolumab & PembrolizumabLung – New way of treating Cancer…Immune checkpoints affect immune system functioning. Immune checkpoints can be stimulatory or inhibitory. Tumors can use these checkpoints to protect themselves from immune system attacks. Checkpoint therapy can block inhibitory checkpoints, restoring immune system function. For example –One such ligand-receptor interaction under investigation is the interaction between the transmembrane programmed cell depth 1 protein (PDCD1, PD-1; also known as CD279) and its ligand, PD-1 ligand 1 (PD-L1, CD274).Many cancer cells express PD1-L1 protein on their surface so that the immune system recognizes this marker as our own cell that should not be killed. Our immune T cell have another protein that is called PD1 or check point protein that involved in the recognition of PD1-L1. When PD1 marker on the immune T cell interacts with PD1-L1 marker on cancer cell the T cell dies or gets inactivated. This way cancer kills our T cells of our immune system and gets stronger whereas our immune system gets weaker. The immunotherapy drugs block the PD1 check point on T cells so that they cannot interact with PD1-L1 on cancer cells and cannot get killed or inactivated. With the blocked check point (PD1) the T cells kill cancer cells without a problem. In summary PD1 is sitting on our T cells and PD1-L1 is sitting on cancer cells. And there is a battle between them. Either cancer cells kills T cell or T cell kills cancer cell.Nivolumab, Pembrolizumab are the new approved drugs for usage in Lung cancer. The immunotherapy drugs are known to be better than Chemotherapy in terms of response and survival.Neuro OncologyHippocampal sparing while treating brain tumors will improve the QOL – Neurocognitive function and memory.Immunotherapy in Brain MetastasesTreatment of brain metastases remains a challenge as most therapeutic agents do not cross the blood brain / barriers. Immunotherapy involves the activation of the immune system in T cells that cross the barrier. Recent studies have shown the efficiency of Pembrolizumab in treatment of Brain metastases in patients with melanoma and Lung Cancers.Head & Neck CancersHPV virus induced Cancers are increasing in our country too. These tumors are mostly in the Oropharynx and throat and are highly curable with less intensive treatment. Immunotherapy in H&N cancers.
