Apollo Hospitals Chennai Achieves Breakthrough with BASILICA-Assisted TAVI, Safeguarding High-Risk Heart Patient

Apollo Hospitals, Chennai, has successfully treated a 67-year-old high-risk heart patient using an advanced, minimally invasive procedure called BASILICA-assisted TAVI. This specialised technique is designed to prevent blockage of the coronary artery during valve replacement. This milestone highlights the growing role of advanced transcatheter therapies in managing complex structural heart disease, particularly in patients who are not suitable candidates for repeat open-heart surgery.  The procedure was performed by a multidisciplinary team led by Dr. Sengottuvelu G, Senior Interventional Cardiologist and Clinical Lead of Structural Heart Interventions at Apollo Hospitals Chennai. This approach offers a minimally invasive alternative for redo valve replacement cases, where conventional surgery carries significant risk. The patient had a history of diabetes, coronary artery disease with prior stenting, and aortic valve replacement surgery in 2017. Degeneration of the previous valve led to recurrent aortic stenosis (narrowing) and regurgitation (leakage), both of which can affect blood flow. Before the procedure, a scan revealed a high risk of blockage (an uncommon but serious complication of TAVI), associated with mortality rates of up to 40–50% when it occurs. To reduce this risk, doctors used a combination of TAVI (a catheter-based valve replacement procedure) and the BASILICA technique. In simple terms, BASILICA involves carefully splitting a part of the old valve using controlled energy so that it does not block blood flow to the heart during the new valve placement.   Dr. Sengottuvelu G noted, “This case highlights the evolving capabilities of transcatheter therapies in managing highly complex structural heart disease. BASILICA is a technically demanding procedure, and its successful execution reflects the expertise and coordination of our team.” Dr. C S Muthukumaran, Interventional Cardiologist at Apollo Hospitals Chennai, added, “Performing the first BASILICA procedure in the Apollo Group is a proud moment. It reinforces our commitment to bringing cutting-edge, life-saving technologies to patients in India.” After the procedure, the patient showed good recovery, with improved blood flow, stable rhythm, and no evidence of heart blockage, and was discharged in stable condition. With international success rates exceeding 90% in selected cases, BASILICA remains a highly specialised procedure, underscoring Apollo Hospitals’ commitment to delivering evidence-based, patient-centric cardiac care.

Apollo Hospitals, Hyderabad successfully treats a critically ill eleven day old baby with a complex and rare ...
Baby of Shaheen (name changed for privacy) had a very abnormal right side heart valve (Tricuspid valve) called Ebstein’s Anomaly, where the valve is abnormal and leaks profusely. He also had atresia of pulmonary valve, which meant that no blood flow was going to his lung from the heart for oxygenation. This life threatening abnormality was detected by the doctors at Apollo Hospitals Hyderabad when the baby was still in the womb through a fetal echocardiography. A medical team at Apollo Hospital, Jubilee Hills Hyderabad, comprising of Paediatric Cardiologist, Obstetrician, Neonatologist and Paediatric Cardiac Surgeon took utmost care to meet any contingency during the birth of the baby considering his precarious medical condition. The baby being born prematurely at 34 weeks of gestation coupled with growth retardation presented a challenge to the team. Initially the team put him on ventilator support soon after birth and hoped to manage him medically for a few months before surgery could be planned. However, a natural connection between his aorta and the artery to the lungs called ductus arteriosus was closing. Hence this connection had to be kept open using a special medicine for taking blood to the lungs for oxygenation. The closure of the artery would have snuffed life out of the baby and therefore it was imperative that Cardiac surgery to restore blood flow to lungs was performed on a war footing. The surgery was especially challenging because of his deteriorating condition, tiny size, small size of the heart (no bigger than an adult thumb) and organs. The task was daunting and the team had to perform the surgery with great dexterity, says Dr Girish Warrier, Senior Paediatric Cardiac Surgeon, who led the team in performing the surgery. After a detailed consultation with the family, the team decided to go ahead with surgery on September 14th, albeit at a high risk, since without surgery, the child would not have survived tiny size. The child had to be placed on Cardiopulmonary bypass as preparatory to working inside the heart. Being only 1.2 Kg at birth and 11 days at the time of surgery, specially designed circuitry had to be used to place the child on bypass. He required an urgent tube graft that would send blood from his aorta to lungs. However, leaving his tricuspid valve leaking so badly would have jeopardized the function of his heart and ultimately the recovery. The leaking tricuspid valve was closed using a patch. Thereafter a shunt was created to provide reliable source of blood supply to the lungs for purification. The child tolerated the procedure well and was taken off the bypass machine and then shifted to the intensive care unit. He gradually recovered from the surgery and was shifted to the ward on the 14th post-operative day. The infant needed a multidisciplinary team approach right from being inside the womb to his recovery that involved Paediatric Cardiologist Dr Kavitha Chintala, Obstetrician Dr. Prameela Sekhar, Cardiac Anaesthetist and Critical Care Specialist Dr Meena Trehan, Paediatrician Dr Sharmila Kaza, residents, fellows, nurses and ancillary staff. Dr Girish Warrier commented,” Cardiac surgery in such tiny children requires an advanced infrastructure and a highly trained staff who form the backbone of any successful team. Additional challenges to his surgery were nutrition and infection control, but he is now recovering well and is planned for discharge in a couple of days from the hospital. This type of surgery is a high-risk surgery which has not been performed till date in India and Apollo Hospitals Hyderabad is proud to be a pioneer in this field”.

Post the launch of the Center of Excellence in Hip Arthroscopy early this year, Apollo Hospitals has successfully treated 5 patients using this procedure. Continuing to excel in Orthopaedics, Apollo Hospitals,Chennai – Indias leading healthcare service provider showcased its expertise in handling complex cases using Hip Arthroscopy procedure. A cutting edge procedure that involves viewing the interior of the hip joint through an arthroscope and then using minimally invasive techniques to treat the disorder, experts believe that hip arthroscopy, will soon gain popularity. 15 year old student from Kolkata came to Apollo Hospitals, Chennai with severe pain in the hip for almost 15 days, which made it difficult for him to perform regular activities like walking. Doctors, here, on further diagnosis identified that he was suffering from synovitis of the hip, for which Hip Arthroscopic synovial biopsy was done. The Hip Arthroscopy procedure, performed in less than a day, enabled mobilization post the procedure. Speaking about the advantages of Hip Arthroscopy, Dr Madan Mohan Reddy, Senior Consultant, Orthopaedic Surgeon, Apollo Hospitals,Chennai said, “Hip arthroscopy provides the advantage of reducing the need for hip replacement procedure, which is quite complex. In addition to this, it is a day care procedure where the patients experience very little pain, minimal blood loss and can also be mobilised quite quickly. People are aware of this procedure and we expect more people to opt for Hip Arthroscopy in the coming months as opposed to open surgery, because of its advantages.”, he added. Hip Arthroscopy is an intricate and technically demanding domain that has been perfected by few healthcare providers and Apollo Hospitals has been successful in leveraging this new technique for the benefit of patients. The institute is equipped with technologies for advanced spinal treatment procedures like Robotic Spinal Surgery, Disc Replacement and Scoliosis Surgery.

Apollo Hospitals observes World Heart Rhythm Week
Apollo Hospitals conducts free consultation for people suffering from Cardiac Arrhythmia from June 6 to June 12, 2011. Cardiac Arrhythmia, or an abnormal heartbeat, is a serious but treatable condition that has affected roughly 8 million Indians in a short period. Hence, Apollo Hospitals feels that free, simple and routine pulse checks would help to save thousands from death or disability resulting from heart rhythm disorders. Apollos Heart Rhythm Week is intended to create awareness and promote better understanding of arrhythmic conditions. Arrhythmias occur when the electrical impulses in the heart, which coordinate the heartbeats don’t function properly, causing the heart to beat too fast, too slow or irregularly. These cardiac rhythm disturbances can occur because of age, hypertension, heart attack, lung and thyroid disease and heart failure etc. Palpitation, dizziness, fatigue, fainting are few of the common symptoms. Detecting arrhythmias at the early stages could improve a patients chance of finding a successful treatment and could save their life. Hence, Apollo is dedicating an entire week for free pulse & BP check and consultation to ensure people to know their pulse and be aware of cardiac arrhythmia. Cardiac arrhythmia can be treated effectively, through drugs, devices, Radiofrequency catheter ablation procedure or a combination of all the three. Apollo Hospitals is one of the few hospitals in India to have most advanced 3D mapping technology [CARTO 3 system]. This technology helps to pin point the site of origin of rhythm disturbances and accurately deliver the Radio frequency energy. Apollo uses this technology to cure complex and life threatening arrhythmias with high success rate. Apollos Heart Rhythm Week runs from 6 to 12 June 2011. Apollos renowned cardiologist & Electrophysiologist, Dr. Karthigesan will do free consultations and pulse checks from 10.00 am to 8.00 pm, the entire week. Around 6 am, a liver had been harvested from a patient declared brain dead in Jawaharlal Institute of Postgraduate Medical Education and Research (JIPMER), Pondicherry and through a well equipped ambulance and a team of doctors, it was flown and needed to be transported to the South of Tamilnadu, Apollo Speciality Hospitals, Madurai immediately so that the transplant surgery could take place without the loss of valuable time. The area is usually heavily congested with traffic. On an average, it takes at least 6 hours to 8 hours to go from Pondicherry to Madurai without traffic. Thanks to the green corridor, the ambulance carrying the liver reached the hospital in a record 3 1/2 hours. The police officers from Vizhupuram, Trichy and Madurai traffic circles and control room coordinated with the Transport Coordinator of the hospital to decide the logistics. While the traffic official (head of a traffic circle) rode in a Jeep as pilot for the ambulance, senior officers monitored the real-time progress of the team on wireless sets. Cops were posted in every intersection along the route to ensure that the ambulance did not have to stop at any point. All traffic lights along the way were turned green for the ambulance, and cross-traffic along the route stopped as the ambulance passed through. Throughout the highway, the loudspeakers and sirens are used to inform other vehicles on the road so that they remained aside while the ambulance passed through.
